The CYMP Program serves Mercer County Youth between the ages of 12-21 who are referred by DCP&P, CMO, JJC and other Children System of Care Agencies.

Group Mentoring – Teen TecK
(Technology-based workshops and classes)

Group Mentoring – Teen Talk
(Monthly Teen Topics- Including TSA/SP with Social Activities using Peer Mentors)

One on One Mentoring – Teen Trust
(One on One Mentoring with a matched Adult Mentor)

Program Objective

To build a solid relationship between the youth and mentor while implementing the Transitional Plan and/or Service Plan. To develop and maintain a group of Peer Mentor and Adult Mentors who are willing to volunteer through mentoring while servicing a community of at-risk adolescents.

Our mission is to empower youth to reach and accomplish their Emotional, Social, Educational and Professional goals.

Vision

We envision a community of once at-risk adolescents to build and develop the social skill set to excel within the community they are living in. To learn to have accountability for one action and learn to trust nurtured relationships with others. Through these skills, learn to develop their full potential.

Youth will be assigned to a mentor selected and matched to the youth’s needs and interest.

Eligibility Criteria:

Youth between the ages of 12 and 21 residing in Mercer County

Youth must be referred by Division for Child Protection and Permanency

Juvenile Justice Commission

New Jersey Children’s System of Care provider and reside in Mercer County.

HOW TO MAKE A REFERRAL:

To make a referral, the Case Manager, Social Worker, Parole/Probation Officer should:

Complete the referral document online then fax to Shiloh CDC

Be sure to include the youth’s current Transition Plan (If applicable)
